      NOTE:  A waiver document is required for every member,  Senior and Juniors (signed by parent or guardian).  Fillable forms that permit electronic initials and signatures have been created for your convenience.  These forms will be emailed to all members for completion (simply click the “Done” button, top right corner when you are ready to submit).  If you do not have an email address on file with BCHBC, please PRINT the appropriate forms below for each family member, complete and submit to your Chapter membership coordinator along with your Membership Application form.
